    I got the short form.
     
    Under “Race” I selected “other” and wrote in “American.”
     
    They know where I live if they need more detail.

    Yup!  That’s IT!  There will be nothing else included.  Although, I do kind of like adding the “other”.  :)   Most of us are muts anyways.
